Dear Nada,
Thanks a million for the this vital post and Iam glad that so many are interested. Thanks to Bakri, I can now add a medical prespective to the debate (will do in arabic asap).It is now widely agreed that prevention, diagnosis and treatment should go hand in hand to control the impending HIV crisis in Africa.
How to do it and where to start?how to prevent a sexually tranmitted diseases and promote sexual health generally where people stuggle to talk about it with their own partners let alone with a health professional??your example about sensorship, published in the BMJ is frustrating but is it really surprising ??societal changes to allow health promotion in sexual health issues is really hard and very slow.Even in the UK where teenage preganacy is the highest in Europe, widely believed to be due to the poor sexual education to girls and boys in schools ,compared to countries like Holand;attempts at openess re sexual issues are often sabotaged by the old guard ( in education, parent groups,church etc)who say such openess, free condoms etc will encourage children to make love, and so they seem to be happy to maintain the status quo at the expence of ,of course, high rate of teenage pregnancies!!,this is how difficult it is.Do it in Sudan!!, and imagine how it would be like,only look at female genital mutilation (more than 90% among medical student U of K,2002) in a recent study, despite more than fifty years of efforts to stop it; it is hard enough thinking about it.
In some countries, life expectancy is being reduced by a decade every year!!.Luckily, this seem not to be the case in Sudan or is it??.You quoted,and also did Muna Obeid, a figure of 1.4% infected in Sudan , i think from the UNAIDS epidemiological fact sheet.However, if you look at the information in that sheet you will soon realize that very little details are available, in fact most of it is empty boxes ie no data/information avaiable.In short, I hope Iam wrong, but i do not think these figure are accurate, as indeed indicated by their authors!!So, to know where one is going we need to know where we are ie a system to monitor and survey HIV that cover all the country. Another issue flgged up in one of the postings, by Eman i think, was that of the new cheap drugs that are now avaiable for HIV, not yet in Sudan, but elsewhere in Africa.Yes, that may be true but HIV drugs are unique in the sense that patients have to take them almost religously without missing any doses, otherwise resistance will soon develope, and patients end up where they started. Also, the HIV patients need to be continuously monitored for response to drug and side effects, often, by some soffisticated tests most of which are not available in Africa.Hence, many in the HIV field questioned wheather rushing drugs to places lacking the basic infrastructure is a wise thing to do

That said,however, most people now agree that some drugs are better than non, both for the individuals ie improve their own health, and for the whole population in terms of reducing spread of the virus, by decreasing the amount of HIV virus in those infected,and thus,stop transmission to others eg from mother to child .There is a lot to be said in this intriguing area, I will come back.
